What is the SNS?

The SNS (Serviço Nacional de Saúde — National Health Service) is Portugal's public healthcare system. It provides medical care through a network of health centres (centros de saúde), hospitals, and specialized clinics across the country.

The SNS covers primary care (family doctor visits), specialist consultations, hospital care, surgery, maternity care, mental health services, and subsidized prescriptions. While not everything is free, the costs are significantly lower than private healthcare.

Who is Eligible?

Access to the SNS has been broadened over the years. The following groups are generally eligible to register:

  • Portuguese citizens and EU/EEA nationals living in Portugal
  • Legal residents — Portuguese nationals and foreign nationals with a valid Portuguese residence permit can register at a health centre, according to gov.pt
  • Children and pregnant women — regardless of legal status, children and pregnant women have access to healthcare

Access rules may differ depending on immigration status. Do not assume that possessing an SNS user number automatically establishes entitlement to every SNS service without charges. Check with your local health centre for your specific situation.

Documents You Need

Bring the following to your local health centre when you register:

  • Identification — Passport, Citizen Card, or residence permit
  • NIF — Your Portuguese tax number. See our NIF Guide if you do not have one yet.
  • Proof of address — Utility bill, rental contract, or a letter from your local júnta de freguesia confirming your address
  • NISS — Your social security number, if you have one. See our NISS Guide.
  • Residence permit — If you are a non-EU citizen, bring your residence permit or proof of your pending application

How to Register: Step by Step

  1. Find your local health centre (centro de saúde) — Each area has a designated centro de saúde based on your home address. You can search online or ask your local júnta de freguesia.
  2. Go in person — Visit the reception desk (secretaria) of your centro de saúde during opening hours. Arrive early, as queues can be long.
  3. Present your documents — Give the staff your ID, NIF, proof of address, and residence permit.
  4. Receive your Utente number — After registration, you receive a card or document with your número de utente (patient number). This is your key to accessing all SNS services.
  5. Request a family doctor — Ask to be assigned a médico de família (family doctor). There may be a waiting list in some areas.

Your Utente Number

The número de utente is your unique patient identification number in the Portuguese health system. You will need this number for:

  • Booking appointments at your health centre
  • Visiting hospitals and emergency rooms
  • Getting prescriptions filled at pharmacies
  • Using the SNS 24 telephone line
  • Accessing the SNS 24 app and online portal

Keep Your Utente Number Safe

Write down your Utente number and keep it in your phone or wallet. You will need it at every medical interaction in Portugal. If you lose it, you can recover it at your health centre or through the SNS 24 app.

Getting a Family Doctor

A médico de família (family doctor) is your primary care physician. They handle general consultations, referrals to specialists, prescriptions, and ongoing health management.

How It Works

  • Your family doctor is assigned by your health centre, based on your address area
  • Appointments are booked through your health centre's reception, by phone, or through the SNS 24 app
  • If no family doctor is available, you are placed on a waiting list and can still use walk-in consultations (consulta aberta) at the health centre

If You Cannot Get a Family Doctor

Some areas have a shortage of family doctors. While waiting, you can:

  • Use the walk-in consultation service at your centro de saúde
  • Call SNS 24 for medical advice and triage
  • Visit an urgência (emergency department) for urgent issues

Using SNS 24

SNS 24 is Portugal's health helpline and digital health platform. It provides medical triage, advice, and can direct you to the right level of care.

SNS 24 Phone Line

Call 808 24 24 24 to speak with a health professional. The line operates 24 hours a day, 7 days a week. Nurses assess your symptoms and advise whether you need a health centre visit, hospital emergency, or home care.

SNS 24 App

The MySNS app allows you to:

  • Book and manage appointments
  • View your prescriptions and medical history
  • Access digital prescriptions
  • Find the nearest health centres and pharmacies

Prescriptions & Medicines

When your doctor writes a prescription (receita médica), it is usually issued electronically. You take the prescription to any pharmacy (farmácia) in Portugal.

  • Subsidized medicines — Many prescribed medicines are partially covered by the SNS. The co-payment percentage depends on the medicine category and the patient's exemption status. Check with your pharmacist or health centre for details.
  • Electronic prescriptions — Most prescriptions are sent directly to the system. Show your citizen card or Utente number at the pharmacy.
  • Pharmacy hours — Most pharmacies are open Monday to Friday, 9:00 to 19:00, and Saturday mornings. Each area has a rotating duty pharmacy (farmácia de serviço) open outside normal hours.

Emergency Services

For medical emergencies, call 112 (the European emergency number). This connects you to police, fire, and ambulance services.

For urgent but non-life-threatening situations, go to the urgência (emergency department) at your nearest hospital. The SNS 24 line (808 24 24 24) can help you decide if you need the emergency room.

Common Mistakes to Avoid

  • Going to the hospital for non-emergencies — Use your family doctor or SNS 24 for routine issues. Emergency departments are for genuine emergencies and urgent cases.
  • Not bringing your Utente number — Always carry it. Without it, administrative staff may not be able to access your records quickly.
  • Forgetting your NIF — Healthcare costs linked to your NIF appear in your e-Fatura for tax deductions. Always give your NIF at medical appointments and pharmacies.
  • Registering at the wrong health centre — You must register at the centro de saúde that serves your residential address area.
  • Not requesting a family doctor — You must explicitly ask to be placed on the waiting list. It does not happen automatically at some centres.

Common Questions

Is healthcare free in Portugal?

Primary care at health centres is available to registered users. Hospital visits and some services may involve co-payments (taxas moderadoras). Certain groups are exempt from co-payments. Check with your health centre for current fees and exemptions applicable to your situation.

Can I choose any doctor?

Family doctors are assigned based on your address. You cannot freely choose a specific doctor within the SNS. However, if you use private healthcare, you can choose any doctor.

What if I do not speak Portuguese?

Most doctors, especially younger ones, speak some English. However, health centre staff (reception, nurses) may only speak Portuguese. Consider bringing a Portuguese-speaking friend, or use the SNS 24 line which may offer support in other languages.

Do I need private health insurance?

Private insurance is not required. However, many residents choose it to access private clinics with shorter waiting times and more specialist availability. The SNS covers essential healthcare needs.
